The Co-Health Group Collegiate Plan has been specifically designed to meet the needs of today's College and University students. Whether you are an incoming freshman, an upper classmen, a graduate student, a part-time student, an adult education student or even a foreign student attending school in the United States for the first time, the Co-Health Collegiate Plan makes a lot of sense and will save you significant dollars on your health care costs. And the areas we have targeted are the ones where you are most likely to incur unexpected expenses during the school year.
The Full Co-Health Benefits Plan provides discounts in certain health care areas not normally reimbursable by Insurance. In the "Collegiate Plan" we are offering the Vision, Dental and Pharmacy Programs as a single package of benefits or they can be purchased separately. Here's how the Plan works:
The Co-Health Group Collegiate Plan is a discount Health Care Plan offering savings, through discounts, for Vision, Dental and Prescription Pharmacy "out-of-pocket" expenses. You choose your selected benefits as you enroll in the program. You will be issued a personal Co-Health Membership Card. Each benefit you select has a network of "Benefit Providers" (for example, the participating dentists in the Dental Plan - 25,000 dentists nationwide). As a member of the Plan, you can go to any of the Providers and purchase their products or services on our specially negotiated discount basis. You get your savings/discounts on the spot. There are no exclusions - there are no "pre-existing" conditions clauses - there are no claim forms to fill out - there are no forms to be submitted - this is not an insurance plan. You simply show your Co-Health Membership Card at the time of your scheduled appointment and the savings belong to you.
The discounts you will receive are substantial and these savings can be very important to you. The programs that make up the Collegiate Plan (Vision, Dental and Pharmacy) are also the three most common health areas where you will have unexpected expenses. With your Co-Health Membership Card, you can substantially reduce your out of pocket expenses. Your membership in the Co-Health Group Collegiate Plan provides benefits to you throughout the United States - anywhere, anytime. A brief description of the specific benefits in the Collegiate Plan follows:
The CO-Health Dental Plan ins the newest concept in Discount Dental Here is how it works. Every procedure a Dentist performs has a specific fee for it, called the Usual and Customary fee (U & C). This fee varies slightly by geographic region in the country and is usually se by a Dental Board. The CO-Health Dental Plan has a fee schedule that is discounted off the U & C schedule. The discounts range from 10% - 40%, saving you important dollars and ensuring proper dental care. Virtually all dental procedures are included in the CO-Health Dental Plan. In addition, we have a patient advocacy desk available to assist you. As an enrolled member you will be able to contact them for assistance in selecting a dentist, making an appointment and they will contact the dentist, on your behalf, to make sure they know you belong to this Plan and ensure your proper savings. All dental specialties are available, including Orthodontics. There are over 50,000 participating dentists throughout the United States. As a member, through the CO-Health Group Collegiate Plan, you will have access to, what we believe is, the finest Discount Dental Plan available in the United States. Pharmacy The Co-Health Pharmacy Plan has two basic components: Retail and Mail Order. Retail - Discounts on brand name drugs range up to 35%. Discounts on generics range as high as 70% of the manufacturer's suggested retail price. Over 58,000 pharmacies throughout the United States participate in our Co-Health Pharmacy Plan. This means that 95% of all the pharmacies in America participate in our network through our PBM (Pharmacy Benefit Management Company). We are electronically online with all these pharmacies - our members receive their discounts and savings instantaneously at the point of sale. The list of participating pharmacies includes thousands of independent pharmacies plus most national and regional chains such as Albertson's, Dominick's, Drug Emporium, Duane Reade, Genovese, Medicine Shoppe, Omni, Pathmark, Rite-Aid, Safeway, Shopko, Value Rite, Revco and Walgreens. Mail Order Pharmacy - Our members may save an additional 10% - up to 45% total by going to our Mail Order Pharmacy for maintenance medications. The Co-Health Pharmacy Program, through our Mail Order Pharmacy, SAV-RX, also offers great buys on Vitamins and Supplements and over the counter items. Most Insurance Co-Pay Plans exclude certain "lifestyle" drugs such as Rogaine, Propecia, Viagra and, in some cases, Birth Control Pills. The Co-Health Pharmacy Program covers all prescription drugs, either at retail locations or through mail order. A Pharmacy will not honor the Co-Health Pharmacy Program if there is a Co-Pay Card also being used. They must honor the Co-Pay Card as Primary Benefit Provider. Co-Health members can go to our network partner's website, www.savrx.com, to review prices for maintenance drugs and over the counter items plus vitamins and supplements, that are sold through our mail order pharmacy.
